What are your office hours?The Main Office is open Monday - Friday, 8:00am - 4:30pm, and Saturdays for scheduled appointments only. The Well Child Clinic is open Monday - Friday, 8:00am - 4:30pm. We are closed for lunch from Noon - 1:10pm.

What is your after-hours contact information?Our After Hours Triage line is: 256-718-9811 Please, note that a $33 fee will be assessed for appointments after 4:30 pm Monday-Friday and on weekends.

Which insurance providers are accepted?Our physicians are participating providers with a wide range of insurance plans, including: Blue Cross Blue Shield (BCBS) UnitedHealthcare (UHC) Aetna Cigna Tricare ALL Kids Medicaid If you don't see your insurance provider listed, please contact our office to verify coverage.

How do I obtain a copy of my child’s medical records?You may request a copy of your child’s medical record by completing and signing our medical record release form (this form is available on our website under PATIENT FORMS and may also be completed in our office). In order to process your request, completed forms may be submitted to us via fax, mail, or in person. Any patient 19 years of age or older is required to sign their own release form. As a courtesy, we will forward a copy of your child’s medical records directly to their new healthcare provider at no cost. Copies given directly to the parent/patient will incur a $5.00 base fee, $1.00 per page for the first 25 pages, $0.50 per page for pages 26-100, and $0.25 per page thereafter.

How do I obtain a copy of my child’s immunization certificate?Our office requires a 24-hour advance notice for the request of immunization certificates. You may call our office to verbally request a copy of your child’s immunization certificate. This certificate may be printed during a visit at our practice. It may also be picked up at our College Street location, or we will be happy to mail or fax it to you directly.

What should I do when I have a form that needs to be completed by my child’s health care provider?Before submitting a form to our office, please, be sure that the parent/patient portion of the form is completed entirely. All forms are completed at our College Street location; therefore, forms may be dropped off, faxed, or mailed to this location. Due to the volume of forms that our office receives, it takes 7 to 10 business days for forms to be completed. It is important to mention that during the months of August and September our office becomes inundated with patient forms for the new school year. This can delay the processing of forms. Plan ahead and do not delay, and submit your child’s form early to beat the back-to-school rush. Patients and parents planning ahead accordingly will help facilitate the process of patient forms in a timely manner.
